(If you want the shirt to be the colors in the picture please state that in the notes)

-  Mascot
- Name under ball (Mom, Stepmom, Grandma, Aunt, Sister, etc.)
- Sleeve name (one sleeve/one word only)
- Number (one player number)
- Design colors (Two colors only/color chart in photos)
